左右两部分:一个正面站立的人形,就是“大”;旁边是“矢”,上端有箭头,下端有箭羽。人被箭所伤,字义由此扩大到一切病痛,这是通行的讲法;也有学者认为“矢”在这里只作声符,字本来写的是“疾速”的“疾”——箭飞得快——“疾病”是借来的用法。到小篆,结构改成《说文》所说的“从疒,矢声”:人形换成病榻的“疒”,箭只留作声符。

Two parts side by side: a figure standing face-on, which is 大, and beside it 矢, an arrow with its head at the top and its fletching at the foot. A person struck by an arrow is wounded, and the sense widens to illness of any kind — that is the usual account. Some scholars instead take 矢 here for a pure phonetic and read the graph as 疾 “swift”, an arrow being fast, with “illness” borrowed onto it later. In seal script the structure changes to what the Shuowen describes as 疒 (the sick-bed radical) with 矢 kept only for the sound.
